This is our opportunity to get to know you. To better prepare you for your initial visit please review the following tips:

  • Please arrive 15 mins prior to your scheduled appointment time to complete any additional paperwork that may be required to better care for you.
  • Always have a copy of your insurance card and driver’s license / legal identification upon check in.
  • Arrive prepared with a copy of any x-rays or medical documentation that may assist our providers during your visit.
  • Take a snapshot with your cell phone or make notes of any current medications including over the counter that you are presently taking including dosage and frequency of use.
  • Be prepared to provide your pharmacy and their contact information should any medications be recommended during your visit.
  • Please note that a 24-hour notification is required to avoid unnecessary cancellation fees.

Insurance Information

At Southern Foot and Ankle Institute, we accept most major health insurance plans, however, to avoid unexpected financial obligations it is extremely important that you contact your insurance carrier to verify coverage and benefits prior to your visit.

Upon arrival for your scheduled appointment, you will be required to present a copy of your insurance card along with your driver’s license / legal identification.

Should you have any changes in your contact information and or your insurance coverage, we encourage you to notify our medical staff as soon as possible so that we are able to accurately update and maintain your personal medical records.

Each and every insurance policy is unique. We strongly encourage each of our patients to contact their insurance provider to obtain clarity regarding their benefits and to obtain a full understanding of their financial responsibility.
